Pharmacist-led interventions in preventing adverse drug reaction in hospital setting

| Abstract | Adverse drug reactions (ADRs) represent a major cause of morbidity, mortality, and increased healthcare expenditure in hospital settings worldwide. Clinical pharmacists are uniquely positioned to identify, prevent, and manage ADRs through a spectrum of evidence-based interventions including medication reconciliation, medication review, therapeutic drug monitoring, patient counseling, pharmacovigilance, and integration of clinical decision support systems. This review comprehensively examines the epidemiology of ADRs in hospitalized patients, the mechanisms underlying their occurrence, and the multifaceted role of pharmacists in their prevention. Evidence consistently demonstrates that pharmacist-led interventions significantly reduce ADR rates, hospital readmissions, and associated costs while improving patient outcomes. Interprofessional collaboration and emerging digital health technologies further augment the pharmacist's capacity to safeguard patients from drug-related harm. Despite challenges including staffing limitations, informatics gaps, and communication barriers, the integration of pharmacist services into hospital care represents a pivotal strategy for enhancing medication safety and patient well-being. |
