Artificial Intelligence Adoption and Competency Development in Healthcare: A Qualitative Exploration of North Indian Hospitals

Abstract This study examines how Artificial Intelligence (AI) is being adopted across clinical and non-clinical domains in hospitals of North India, and how healthcare personnel are preparing to engage with these technologies. Drawing on in- depth interviews with doctors, administrators, and IT staff from selected hospitals, it explores the lived experiences, expectations, and concerns surrounding AI integration. Thematic analysis revealed seven dominant patterns: clinical and non-clinical AI deployment, training and upskilling imperatives, diagnostic efficiency, ethical concerns, psychological resistance, infrastructural limitations, and trust in AI systems. Participants consistently highlighted that while AI has improved diagnostic turnaround time and reduced repetitive tasks, its benefits are unevenly realized due to generational differences in digital fluency and gaps in structured training. Younger professionals adapted more readily, whereas senior staff often expressed anxieties about transparency, data privacy, job displacement, and financial costs of implementation. Trust and explainability emerged as critical to overcoming resistance. Findings underscore the need for tailored training programs, transparent AI interfaces, and interdisciplinary governance frameworks to ensure equitable adoption. The study concludes that sustainable AI integration in Indian healthcare depends not only on technological readiness but also on workforce preparedness, ethical safeguards, and supportive policy mechanisms. By situating AI adoption within the sociotechnical realities of North India, the research provides empirical insights into how employee competency shapes and constrains digital transformation in healthcare, offering practical implications for administrators, policymakers, and educators seeking to align innovation with human capability.
