The Paradox of Productivity: A Clinical review of High Functioning Depression

| Abstract | High-functioning depression (HFD) represents a "silent" demographic characterized by individuals who maintain high occupational and social performance while experiencing persistent depressive symptoms. This paper explores the diagnostic challenges of HFD, which often aligns with persistent depressive disorder (PDD) but escapes recognition due to preserved functional capacity. Utilizing a mixed-methods approach (N=50) with the PHQ-9 and WSAS tools, the study identifies subjects with high depressive scores but low functional impairment. Findings highlight the neurobiological state of functional hyper-arousal and the psychological toll of "masking," perfectionism, and contingent self-worth. The paper concludes that clinical reliance on functional impairment as a primary indicator is insufficient and underscores the risk of a "sudden crash" or catastrophic break if left untreated. |
